The Clinical Pharmacist works as part of the multidisciplinary team to improve patient outcomes, optimise medicines use, support long-term condition management, and reduce GP workload. The postholder provides expert clinical advice, conducts medication reviews, supports prescribing safety, and contributes to high-quality, patient-centred care across the practice.

We are looking for an enthusiastic and proactive individual to join our busy team. This is an exciting time for the practice and the successful candidate will have a fantastic opportunity to use their skills to further develop this role whilst providing a high standard of care for our patients.
