Clinical Pharmacist

The Clinical Pharmacist works as part of the multidisciplinary team to improve patient outcomes, optimise medicines use, support long-term condition management, and reduce GP workload. The postholder provides expert clinical advice, conducts medication reviews, supports prescribing safety, and contributes to high-quality, patient-centred care across the practice. We are looking for an enthusiastic and proactive individual to join our busy team. Job responsibilities

• Conduct structured medication reviews for patients with polypharmacy, frailty, multimorbidity, or high-risk medicines.
• Review and manage repeat prescriptions, ensuring safe and cost-effective prescribing.
• Provide clinical support for all long-term conditions, including hypertension, asthma, COPD, diabetes, cholesterol, Atrial fibrillation, etc. within the scope of competence.
• Undertake medication reconciliation following hospital discharge, outpatient appointments, or care transitions.
• Identify and manage medicines-related risks, including monitoring requirements and safety alerts.

Prescribing & Deprescribing

• Prescribe and deprescribe medicines as an independent prescriber, working within clinical competence and practice protocols.
• Support GPs with complex prescribing queries and therapeutic decision-making.
• Implement evidence-based prescribing initiatives aligned with ICB and national guidance.

Patient-Facing Clinical Work

• Run clinics for medication reviews, long-term condition support, and medicines education.
• Provide advice to patients on the safe and effective use of medicines, including adherence and side-effect management.
• Support self-care and shared decision-making.
• Participate in all vaccination clinics for example flu, Covid etc.
• Undertake home visits to care and nursing homes including community when needed.

Quality, Safety & Governance

• Lead on medicines audits, QOF indicators, IIF requirements, and prescribing quality improvement projects.
• Implement MHRA drug safety updates and ensure practice compliance.
• Contribute to antimicrobial stewardship programmes.

Practice Integration

• Work closely with GPs, nurses, HCAs, social prescribers, and other ARRS roles.
• Provide training and support to practice staff on medicines management processes.
• Participate in MDT meetings, case reviews, and clinical discussions.

Administrative & Operational Duties

• Manage medication queries from patients, community pharmacies, care/nursing homes and secondary care.
• Support repeat prescribing systems, synchronisation, and workflow optimisation.
• Maintain accurate, contemporaneous clinical records in line with professional standards.
• Complete workflow documents from Docman as per KPIs.
• Be responsible for all safety alerts.
• Run searches and perform audits as needed.

Qualifications

• Masters degree in Pharmacy (MPharm) or equivalent.
• Registration with the General Pharmaceutical Council (GPhC).
• Completion of, or enrolment in, the CPPE Primary Care Pharmacy Education Pathway (PCPEP) (if required under ARRS).
• Evidence of ongoing CPD.
• Postgraduate clinical qualifications (e.g., clinical diploma, advanced practice modules).

Knowledge & Skills

• Strong clinical knowledge of therapeutics, long-term conditions, and medicines safety.
• Ability to interpret and apply national guidelines (NICE, MHRA, local ICB formularies).
• Excellent communication skills with patients and clinicians.
• Ability to work autonomously and as part of a multidisciplinary team.
• Strong organisational and time management skills.
• Competence in clinical documentation and digital systems.
• Skills in audit, quality improvement, and data analysis.
• Understanding of population health management and risk stratification.

Experience

• Experience delivering clinical pharmacy services in primary care, community, or hospital settings.
• Experience conducting medication reviews and providing patient-facing consultations.
• Understanding of QOF, IIF, NICE guidance, and NHS primary care structures.
• Experience working in a GP practice or PCN.
• Experience with EMIS.
• Experience leading medicines optimisation projects.
