An exciting opportunity has arisen to be part of the nursing team at St. Rocco's Hospice. We are looking to recruit a Palliative Virtual Ward and Hub staff nurse. The successful applicant should be an enthusiastic nurse who strives to ensure high standards of patient care are maintained in accordance with St Rocco's Hospice policies, procedures and guidelines.
Please specify in your application your preferred working pattern/hours. Hours will be worked between Mon to Sun, including days, nights, evenings and weekends, on a rota system, following discussion and agreement with your line manager.
Unsociable Hours Enhancement
Hours worked at the following times will be paid as follows: Monday to Friday from 8pm until 8am - £22.10 per hour, Saturday - £22.10 per hour, Sunday and Bank Holiday - £27.20 per hour - T&Cs apply. Shift patterns will be discussed at interview, breaks are unpaid.

Main duties of the job
Reporting to the Ward Manager, Clinical Lead for IPU the main purpose of this role is to provide the highest standard of physical, psychological, social and spiritual care for patients, their relatives and friends. You will use your expertise to the highest standards in order to meet all the requirements of our clinical governance.
Applicants must be on the NMC register and be fully qualified RGN / Adults. In addition to your proven clinical skills, we are looking for excellent communication skills, empathy and sensitivity, good team working and a flexible approach to the role.

Job responsibilities
- To support the Clinical Lead Outreach Services where required with the development and delivery of the strategic business plan objectives aligned to the strategy of the hospice and health and social care system.
- To manage the day‐to‐day patient caseload and work allocation of the Palliative Care Virtual Ward. This will include responsibility for assessment planning, implementation and evaluation of individual patient goals and coordination of the daily MDT meeting.
- To undertake first assessment visits and ongoing monitoring in line with agreed care plan in line with evidence‐based practice.
- To work under the direction of the Ward Manager to ensure that the climate of the service is conducive to patients/relatives spiritual, psychological and physical well being.
