Our Living Well Centre offers a programme of activities and interventions designed to support patients and carers physically, emotionally, socially and spiritually. As a Living Well Assistant, you will play a central role in ensuring the Living Well Centre runs smoothly and that every person feels welcomed, supported, and empowered.
Responsibilities:
- Supporting patients and carers during group activities and individual interventions.
- Liaising with the multi‑disciplinary team to ensure co‑ordinated, high‑quality care.
- Working autonomously, where appropriate, to meet individual patient and carer needs.
- Providing empathetic, clear communication and telephone support.
- Engaging with our community to raise awareness of the Living Well Centre.
- Conducting occasional home‑based assessments as needed.
- Ensuring a warm, inclusive environment for everyone who attends.
- Providing administrative support including clinical record‑keeping and referral processing.
- Contributing to service development, projects and audits.
Requirements:
- Strong communication and interpersonal skills.
- Ability to work both autonomously and as part of a multi‑disciplinary team.
- Compassion, emotional resilience and a person‑centred approach.
- A flexible, proactive and positive attitude.
- Good organisational, IT and time‑management skills.
- A full driving licence and access to a car for business use.
- Willingness to work towards a Level 3 Health & Social Care qualification (if not already held).
- Experience as a Health Care Assistant, Therapy Assistant or similar.
- Experience supporting people in a group setting.
- Experience in palliative care, community development or working with volunteers would be an advantage.
