Key Results

Summary responsibilities:

  • Occupational Therapy assessments of clients in our 13 D2A pathway 2 beds.
  • Setting and reviewing rehab goals and rehab programmes, determining accurate discharge dates and post-discharge equipment and care requirements, and ordering equipment from the local NHS community equipment stores (Medequip).
  • Attending MDT meetings.
  • Home visits.
  • Working in a team with other OTs, physios and support staff.
  • Assessing permanent residents of the care home and residents in the retirement village to reduce the risk of falls, support rehabilitation, posture management, and promote movement, independence, social engagement, and general wellbeing.
  • Implementing therapy treatments including ADL training, equipment provision and training, education, transfers education/practice.
  • Residents-centred multidisciplinary team working.
  • Helping residents to manage acute and long-term conditions.
  • Safely and appropriately delegate therapy tasks to therapy assistants.
  • Maintain accurate records necessary for the role.
  • Communicate readily and effectively with residents, their families, colleagues, and other individuals from outside the organisation.
  • Take an active role in the development of the Therapy Service.
Skills, experience and qualifications

Essential:

  • BSc (Hons) Occupational Therapy.
  • Member of RCOT and registered with HCPC.
  • Good IT skills.
  • Experience in working with older people - including frailty and dementia care.
  • 2 years post-graduate experience.
  • Experience working with therapy assistants.
  • Experience working in NHS.
  • Up-to-date professional knowledge.
  • Evidence of personal development.
  • Excellent team worker.
  • Motivated and enthusiastic worker.
  • Orthopaedics experience.
  • Community/home visit experience.
  • Enthusiasm for improving the wellbeing of older people.
  • Approachable.
  • Experience in hospital discharge planning and rehabilitation.

Desirable:

  • Experience of working within a care home.
  • Experience working in BNSSG, especially North Somerset.
  • D2A experience.
  • Experience ordering equipment using TCES Community.
  • Experience in wheelchairs and/or seating.
  • Accredited referrer for BWCS.
  • Neuro experience.
  • Driving license and a car.
