We're looking for a highly motivated, passionate Registered Nurse to join our expanding Hospice at Home Team at St. Michael's Hospice. This is a specialist nursing role, providing compassionate, expert care to patients in the last weeks and days of their lives. An ideal opportunity for a nurse who wishes to specialise in palliative and end of life care, or who already has experience in this area.

Someone who has a passion for making a difference to patients and their families alongside a love of nursing are some of the key aspirations we are looking for in the successful candidate. Experience of working within the community setting would be an advantage. The successful candidate will be an NMC registered nurse with an active PIN. You will have excellent communication skills and empathy.

You'll work both independently and as part of a supportive multi-disciplinary team, proactively assessing patients' needs, planning and delivering high-quality care, and advocating for the best possible outcomes. The role involves working shifts, including both long days and night shifts (07:30-20:00 / 19:30-08:00), over a 7 day week. A full UK driving licence and access to a vehicle are essential, as you will be travelling across the community within the catchment area.

Main duties of the job: The 24/7 service will offer a visiting service to patients within our catchment known to be in the last 2 weeks of life. Working collaboratively with the wider multi-disciplinary team, you'll assess, plan, and provide holistic care that meets the physical, emotional, and spiritual needs of patients. You'll also play a key role in contributing to service improvement and promoting best practice in end-of-life care.
