About The Role

We are looking for compassionate, organised and resourceful people to join our Social Work, Information and Advice Team. Working alongside social workers, you will often be the first point of contact for patients and families, providing practical advice and signposting across community services. You will support patients, carers and families with applications for benefits, grants and other assistance; coordinate referrals; liaise with partner agencies; and connect people with local services and activities in order to maintain independence, wellbeing and quality of life.

Working under the guidance of social workers and alongside members of the multi-disciplinary team, you will be able to manage referrals and follow‑up actions efficiently. You will recognise and elevate safeguarding concerns and handle records and confidential information in line with GDPR, and hospice policies. You will also have, or be able to develop, a good knowledge of local community resources, voluntary services and referral pathways.

This new role is an exciting opportunity to play a vital part in helping people access practical support during challenging times. You will join a collaborative team, build strong community connections and make a visible difference to the experience of patients and carers.

***We are looking to recruit 3 roles, 1 x 30 hours per week, and 2 x 22.5 hours per week. Interviews will be held for successful candidates on Week Commencing the 23rd of November 2026***
