Job Responsibilities

• Build a natural and trusted rapport with patients and families.
• Provide compassionate care and support through home visits and telephone advice.
• Support people with sensitive topics including loss, grief and dying.

Qualifications

• Excellent communication skills and sound judgement.
• Commitment to patient‑centred interdisciplinary team working.
• Palliative care experience desirable but not essential.
• DBS check and valid UK driving licence required.
