At St Michaels Hospice, we are committed to providing the highest quality of care to our patients and their families. This is an exciting opportunity to join our integrated community nursing team and be part of our evolving community model providing advice and support to patients in their own homes.

To support the holistic care of people with specialist palliative care needs in the community through the provision of expert palliative care assessment and management, in line with their personal priorities and goals. To act as a specialist resource for the patient, their families and the wider health and social care team in the support and care of people with life limiting illness including physical, psychological, social or spiritual support needs.

As a senior member of the Integrated Clinical Team, to play an active role in clinical governance, clinical service development, audit and research, including leadership of clinically facing projects. To work in an integrated way, fostering strong working relationships and effective communication to ensure seamless support and care for patients and families. To support 7-day service provision and participate in the Clinical On-call rota.

We are looking for someone who is passionate about palliative care and has transferable nursing skills and experience. You will be able to build a natural and trusted rapport with patients and families and provide compassionate care and support to those who need it most. Our ideal candidate has excellent communication skills, sound judgement, and a real commitment to patient-centred interdisciplinary team working. You will be able to openly and honestly support people with sensitive topics including loss, grief and dying.

Palliative care experience is desirable, but it is not essential to join our Hospice team if you have transferable experience. We invite you to spend time with us, meeting the team, seeing the hospice and gaining insight into how you can make a difference to patients and their families by joining St Michaels Hospice.

Our services are provided via in-patient beds and at home and we also offer Wellbeing support, both out-patient and virtual.
