Are you an experienced and compassionate nurse looking to make a real difference? We have an exciting opportunity to welcome a Clinical Nurse Specialist to join our Community Nurse Specialist team, based in and working throughout Taunton. You will undertake the holistic assessment, planning and delivery of specialist palliative and supportive care for patients with advanced, progressive life‑limiting disease.
Salary & Contract Details
- Salary: £39,920.27 – 48,070.94 per annum (pro rata if part time)
- Additional Payments: 30% unsocial hours on Saturday and 60% on Sunday & bank holidays
- Working Pattern: Up to 37.5 hours per week available
- Contract: Permanent
Responsibilities
- Managing and prioritising a specialist caseload of patients in the community
- Undertaking holistic assessments and developing personalised care plans
- Providing expert symptom control and end‑of‑life care advice
- Supporting patients and families with advance care planning
- Working collaboratively with multidisciplinary teams and primary care colleagues
- Delivering sensitive and complex communication with compassion and professionalism
- Maintaining accurate clinical records and contributing to quality improvement
- Supporting education, mentorship and development of colleagues and students
Qualifications
- Be a Registered Nurse
- Have post‑registration experience within palliative care, oncology, or a related subject
- Be able to work autonomously and as part of a team
- Have excellent communication skills
- Be passionate about palliative care and have a thorough knowledge of life‑limiting illnesses
As this role is based in the community, the ability to travel is an essential requirement.
