Were looking for a skilled, empathetic, and motivated nurse to join our dedicated community team, delivering exceptional care and support to patients in their own homes, care homes, and community settings., As a St Lukes Community Nurse, you will play a vital role in providing specialist palliative care. This is a varied, rewarding position that blends clinical excellence with holistic care. Working as part of a multidisciplinary team, you will assess, plan, and deliver tailored care for patients with progressive and life-threatening conditions. Youll support patients in achieving their preferred place of care, help prevent avoidable hospital admissions, and provide compassionate guidance for families and carers. This role offers opportunities to develop specialist skills, support students and junior colleagues, and be involved in education and training. You will work under the clinical leadership of advanced practitioners but will be empowered to act with autonomy and clinical decision-making.
