St Luke’s Hospice in Sheffield is seeking a Specialist Palliative Care Social Worker to join our multidisciplinary team on a 12-month maternity cover. You will provide expert social work assessment, guidance and support to people living with life-limiting conditions and their families, across inpatient, outpatient and community settings.
You will manage complex cases, advocate for benefits and access to services, and ensure safeguarding and DoLS/MCA compliance while contributing to discharge.
