St Luke’s Hospice cares for people approaching the end of their life in Harrow and Brent. We aim to give the people we care for the best possible quality of life, each and every day. Supporting local people and their families is at the heart of what we do – focusing on what matters the most to them is at the centre of our care.

We receive around 40% of our funding from the NHS and the rest is raised from the generosity of organisations and people who make our care possible. Our local community is a vital partner in our care, kindly supporting us through their donations and fundraising alongside the dedication of volunteers who freely give their time to support the best possible end-of-life care for local people.

The Role

St Luke’s Pall24 service offers round-the-clock, compassionate support to individuals with life-limiting illnesses, their loved ones, and fellow healthcare professionals. We are seeking a Senior Nurse or Paramedic—or a committed Registered Nurse ready to take the next step in their career—to lead urgent frontline responses, provide specialist telephone advice, and deliver hands-on care in crisis situations.

What You’ll Be Doing

  • Triage urgent referrals and calls with confidence and clinical judgement
  • Provide telephone advice and arrange care packages in partnership with Hospice at Home services
  • Make crisis home visits to assess, support, and manage urgent needs
  • Coordinate with health and social care partners, ensuring seamless patient-centred care
  • Supervise and support Healthcare Assistants in delivering high-quality community-based palliative care

What We’re Looking For

  • Registered Nurse (NMC) or Registered Paramedic (HCPC)
  • Minimum 2 years’ recent community or palliative care experience
  • Proven ability to manage complex clinical situations, both remotely and face-to-face
  • Strong knowledge of palliative and end-of-life care pathways
  • Confident with lone working and able to make safe, informed decisions under pressure
  • Car driver/owner essential
