St Luke's Hospice in Basildon is seeking a Rapid Access to discharge (RADS) Nurse Clinician to join our Hospice Community Services Team. You will undertake holistic assessments and coordinate palliative and end-of-life care for patients nearing the end of life, organising personal care with domiciliary providers or sourcing 24-hour care in the home with families and carers.
You will collaborate within a multidisciplinary team to deliver holistic care across settings, support carers and families.
