St Luke's Hospice is a local charity providing compassionate care to people whose illnesses are no longer curable. We promote dignity in dying and empower people to make the choices they want, from the moment they are diagnosed. We are seeking a skilled and compassionate Registered Nurse to join our Hospice Rapid Access Service (HRAS). This role is 22.5 hours per week over 3 days working in accordance with the Department Rota.
The post holder will be required to have a full driving licence, use of a car with business car insurance, to visit patients in the community.
About the Role
The postholder will work with Hospice Services and external partners to assist in the assessment and placement of funding for palliative and end of life care patients to meet their current and ongoing needs including assisting people to go home with the appropriate domiciliary care or 24-hour placement if appropriate. The Registered Nurse working for the Hospice Rapid Access service will undertake thorough patient assessments in the community or in an inpatient setting. This role requires excellent clinical reasoning, multidisciplinary collaboration and the ability to communicate sensitively with patients and carers.
The post holder will demonstrate excellent communication and negotiation skills as well as a sound knowledge base linked with holistic assessment and advanced care planning. They will be an effective team member whilst working autonomously as required. They will have a commitment to collaborative working actively supporting and liaising with other health, social and voluntary care professionals and agencies.
Salary £32,632.13 to £41,563.99 per year (depending on experience, pro rata)
