An exciting opportunity has arisen for an enthusiastic, well-motivated professional to specialise in end-of-life care.

Main duties of the job:

  • St Luke's is delivering on its vision of a Hospice without Walls with the support of its End-of-Life Urgent Care Team.
  • As a Registered Nurse or paramedic for the End-of-Life Urgent Care Service, you will assist in supporting community-based end of life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.
  • You will need to be able to work autonomously and be responsible for ongoing holistic assessments and planning of care to meet the patients' practical, psychological, spiritual, and social needs.
  • You will work closely with both primary and secondary care health professionals and other Hospice colleagues; therefore, good communication skills are essential.
