A 1-Year Fixed-Term Contract with hours of 34.5 per week and a salary range of £30,273-£36,851 FTE depending on experience.

An exciting opportunity has arisen for an enthusiastic, well-motivated professional to specialise in end-of-life care. St Luke's is delivering on its vision of a 'Hospice without Walls' with the support of its End-of-Life Urgent Care Team. As a Registered Nurse for the End-of-Life Urgent Care Service, you will assist in supporting community-based end of life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.

To be a part of this caring and compassionate team as a Band 5 Nurse, you will need to be able to work autonomously and be responsible for ongoing holistic assessments and planning of care to meet the patient's practical, psychological, spiritual, and social needs. You will work closely with both primary and secondary care health professionals and other Hospice colleagues; therefore, good communication skills are essential.
