St Luke’s Hospice Plymouth is seeking a Band 5 Nurse/Paramedic to join the End-of-Life Urgent Care Service, delivering compassionate community-based EOL care and supporting patients in their preferred place of care or death.
You will conduct autonomous holistic assessments and plan care to meet patients’ practical, psychological, spiritual and social needs, while collaborating with primary and secondary care professionals and hospice colleagues.
