An exciting opportunity has arisen for enthusiastic, well-motivated professionals to specialise in end-of-life care.

Main duties of the job:

  • As the Band 6 Deputy Clinical Lead, you will be supporting the Clinical Lead for EOLUCS to manage the operational management of the EOLUCS team whose aim is to support community-based end of life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.
  • The post holder will support the clinical decision making for those patients under the care of EOLUCS, undertaking specialist holistic assessment of patient/family needs in partnership with the patient and their family/carer as required.
  • Developing and being responsible for networking and building relationships with both internal and external health and social services to ensure patients receive appropriate joint up care as part of a system wide approach to EOL care within the catchment area we serve as a Hospice.
