  • Supporting patients and families: Provide sensitive, compassionate support to patients, relatives and carers throughout illness and end of life care, maintaining confidentiality and building trusting relationships.
  • Promoting safety and quality: Ensure medications, treatments and clinical interventions are delivered safely and in line with Hospice policies, procedures and professional standards.

Maintain accurate and timely clinical records using Systm One.

  • Coordinating care: Work in partnership with Hospice and community teams to ensure patients receive appropriate support and wherever possible, avoid unnecessary or inappropriate hospital admissions.
  • Supporting and developing colleagues: Contribute to the learning and development of colleagues, support students and new team members, and participate in clinical supervision, reflective practice and team debriefs.
  • Maintaining professional development: Keep up to date with current research and evidence based practice, attend mandatory training and study days, and contribute to clinical audits and service improvement.
  • Taking responsibility: Support the day to day running of Clinical Outpatients, taking charge of a shift when required and helping to maintain high standards of care, safety and teamwork.
