Hospice Rapid Access Service (HRAS) Care Coordinator
We are seeking a compassionate and organised individual to join our Hospice Rapid Access Service (HRAS) as a Care Coordinator. This role is vital in ensuring that our patients receive the highest quality of care by coordinating and managing their healthcare plans.
Location: Basildon Hospice
Salary: £27,000 - £30,100 depending on experienceHours of work: Full-Time 37.5 hours per weekContract: Permanent
Key Responsibilities for Care Coordinator
The primary duties of the Care Coordinator include:
- Developing and managing comprehensive care plans for patients.
- Coordinating with healthcare professionals to ensure continuous and consistent patient care.
- Monitoring patient progress and adjusting care plans as necessary.
- Providing support and guidance to patients and their families regarding healthcare needs and services.
- Maintaining accurate and up-to-date patient records.
- Organising and scheduling appointments, treatments, and follow-ups.
- Facilitating communication between patients, families, and healthcare teams.
- Ensuring compliance with all healthcare regulations and standards.
Qualifications and Experience Required
We are looking for candidates who demonstrate the following:
- Previous experience in a healthcare or care coordination role.
- Strong organisational and multitasking abilities.
- Excellent communication and interpersonal skills.
- Proficiency in Microsoft Office applications (Word, Excel, Outlook).
- Ability to work both independently and as part of a team.
- Knowledge of healthcare regulations and best practices (desirable).
