Position Overview As a Home Health RN, you will provide in-home care for patients who have been recently diagnosed, discharged from the ER, or require ongoing respiratory treatment, wound care, or medication management. You’ll conduct home visits to monitor patients’ progress with their care plans, educate them on their conditions, and assess both their physical and emotional well-being. When patients are ready for discharge, you will coordinate resources and educate both the patient and their family, ensuring they have everything they need to continue their recovery at home.
What You Can Expect from This Role Flex Schedule
: Required to work one shift per pay period. Days of the week will vary Mon-Fri 8:30 AM to 5:00 PM, may include evening on-call and weekend shifts Low Visit Requirements
