About the Role

We are looking for a qualified nurse to join our experienced Hospice at Home team. If you are compassionate and driven to make a difference to patients and their loved ones at a difficult time in their lives, and looking for a role working within the community to provide skilled nursing care for patients with palliative and end of life care needs, please get in touch.

Responsibilities

  • To provide skilled nursing care for patients with palliative and end-of-life care needs and their family in a community setting.
  • To raise clinical standards and promote a learning environment for other health care providers.
  • Be responsible for planning, assessing, implementing and evaluating patient care.
  • Works effectively with community professionals, support services and members of the primary health care and palliative care teams.
  • To support and direct Registered Nurses, Assistant Practitioners and Healthcare Assistants in implementing individualised plans of care.
  • To lead and coordinate shifts, managing the team of Registered Nurses, Assistant Practitioners, Healthcare Assistants and volunteers, ensuring future shifts are adequately covered with staff according to the changing needs of the service.
  • Support and contribute to an environment of continuous quality improvement.
  • Work flexibly to meet service demands, which will include unsocial hours, weekends, bank holidays as part of the nursing rota, as well as rotating to other appropriate patient care areas within the organization as required.

Qualifications

The ideal candidate would have previously worked in a community setting; however, we would welcome applications from experienced individuals without community service.

Shift pattern: 7am - 3pm (early) or 2pm - 10pm (late). No night shifts are required. Hours will be worked over 4 days across a 7 day rota.

Benefits

£32,448 to £33,446.40 a year based on 30 hrs/week, dependent on pension scheme.
