Act as the first point of contact for patient queries working in the triaging hub, managing minor ailments, acute medication requests, structured medication reviews & QOF work.

Responsibilities

  • Acute Triage and Minor Ailments: Assessing, diagnosing, and managing minor illnesses, including telephone triage and providing advice, or signposting to appropriate services (e.g., community pharmacy).
  • Clinical Medication Reviews: Conducting structured medication reviews (SMRs) for complex, long‑term conditions (e.g., hypertension, diabetes) and managing polypharmacy.
  • Prescription Management: Handling acute prescription requests, repeat prescription reviews, and managing medication changes for patients recently discharged from hospital.
