St Helens South Primary Care Network
Location: St Helens
Contract: 12 Month Fixed Term (3-month probation)
Sessions: Up to 4 sessions per week (flexible by agreement)
Sessional Rates: Competitive
Employment Type: PAYE or Self-Employed
Working Arrangements: This role is up to 4 sessions per week, with sessions agreed in advance. Some flexibility will be required to attend system meetings or project groups when needed. We can consider candidates wanting to be either employed on a self-employed or PAYE basis.
Main duties of the job:
- Clinical Leadership: Provide clinical leadership and oversight for neighbourhood projects. Ensure initiatives are clinically safe, evidence-based and patient-centred. Support the design, implementation and evaluation of neighbourhood models of care. Act as a clinical reference point for neighbourhood service development.
- Project Delivery: Provide clinical oversight for neighbourhood programmes and projects. Work with the PCN Operations Manager to ensure delivery against milestones and outcomes. Identify and manage clinical risks related to neighbourhood initiatives. Contribute to reporting on project progress, outcomes and impact.
- System & Place Working: Represent the PCN in Place-based meetings and system groups. Work collaboratively with partners across primary care, community, voluntary and acute services. Participate in task and finish groups supporting neighbourhood priorities. Support the Operational Clinical Lead with staff PDRs, appraisals and supervision. Promote a positive learning culture and support staff development. Provide clinical advice and leadership support across the wider PCN team.
Person Specification:
- Qualifications: Registered clinician (e.g. GP, Advanced Clinical Practitioner, Nurse or Allied Health Professional) with current professional registration.
- Experience: Experience working within primary care or community health services. Experience contributing to service development or quality improvement projects. Experience of clinical leadership within a PCN or Place setting. Experience of project management or leading service change. Experience supporting staff development or appraisal processes.
- Other skills: Strong communication, leadership and collaborative working skills. Commitment to patient safety, quality and equitable care. Knowledge of NHS neighbourhood, Place and integrated care agendas.
