St Helens South Primary Care Network (PCN) is seeking an experienced and highly motivated Band 6 (band 5 considered) Mental Health Nurse to join our multidisciplinary primary care team.
This is an exciting opportunity to work at the forefront of integrated primary care, delivering high-quality mental health assessment, intervention and care coordination for adults with a range of mental health needs. You will work autonomously within your scope of practice while contributing to service development.

Key Responsibilities
- Provide comprehensive mental health assessments, formulation and care planning for patients presenting in primary care
- Deliver evidence-based interventions for common mental health conditions (e.g. anxiety, depression, PTSD, severe mental illness)
- Undertake risk assessments, crisis management and safeguarding interventions
- Manage a defined caseload and work autonomously with supervision as required
- Liaise with GPs, multidisciplinary teams, and secondary mental health services to ensure coordinated care
- Support long-term condition management for patients with serious mental illness
Leadership and Service Development
- Provide clinical supervision, mentorship and support to junior mental health staff and trainees
- Contribute to service development, quality improvement initiatives and PCN mental health pathways
- Participate in audits, policy development and clinical governance activities
- Act as a mental health resource and advisor within the PCN
Education and Health Promotion
- Deliver psychoeducation to patients, carers and families
- Support mental health promotion, prevention and early intervention initiatives across the PCN population
About You
- We are looking for a confident and compassionate Registered Mental Health Nurse with strong clinical and communication skills and experience working in community or primary care settings.
- Registered Mental Health Nurse (RMN) with current NMC registration
- Significant post-registration experience in mental health (community, primary care or secondary care)
- Experience of autonomous clinical practice and caseload management
- Strong assessment, risk management and safeguarding skills
- Excellent communication, interpersonal and organisational skills
- Experience working in a Primary Care Network or GP practice setting
- Postgraduate qualification or training in mental health, CBT-informed interventions or psychological therapies
- Experience in leadership, supervision or mentorship
- Non-medical prescribing qualification or working towards
