St Helens Borough Council is recruiting a 21 hour Outreach Support Worker to complement our day support offer for adults with learning disabilities, autism and dementia. The role covers home visits and day services across the borough, providing practical support and monitoring wellbeing.

You will assist with appointments, make welfare calls, record activity electronically and participate in support planning. A driving licence and DBS clearance are required, with training provided where needed.
