We are seeking to recruit a 21 hour Outreach Support Worker to complement our existing day support offer to adults with learning disabilities, Autism and dementia. This is an exciting new opportunity for those who thrive on variety and enjoy a challenge.
Responsibilities
- Support individuals with learning disabilities, Autism and dementia at home and within the day service.
- Assist with medical appointments and make telephone welfare calls.
- Work autonomously across the borough, adapting to changing needs of users and the service.
- Record activity electronically, attend meetings and contribute to support plans.
Qualifications
- Minimum NVQ Level 2; further training provided as appropriate.
- Full DBS clearance.
- Full driving licence.
- Strong interpersonal skills with users, carers and professionals.
- Ability to work independently, collaborate within a team and maintain accurate records.
