Job Description:
We are St Helens Council, located in the heart of the Northwest close to Liverpool, Manchester, and Lancashire. As part of our transformation journey, we are seeking an experienced HCPC registered Occupational Therapist to join our team. The role involves providing expertise in assessments and support for adults with disabilities, including complex physical disabilities and mental health conditions such as dementia. Responsibilities include conducting specialist assessments, facilitating discharge planning, supporting smooth transitions into care homes, and working within an integrated team to ensure timely hospital discharges.
