Key to this vision is Contact Cares; which aims to simplify the way we access health and social care services from nursing assessment to social care; reablement to audiology, safeguarding to occupational therapy. Our Occupational Therapy Team are committed to supporting and assisting people with disabilities to improve their quality of life and be as independent as possible in their home environment.

You will be responsible for triaging referrals received to the Occupational Therapy Team and deal with a variety of queries that come in relating to the Occupational Therapy Service. You will need to interact with service users, family members and professionals in order to gain more information to support the triage process. You should be self-motivated and have excellent listening and communication skills, both written and verbal and be able to confidently build rapport with service users and family members over the phone. You should be competent in accessing IT systems such as Microsoft Word and Excel and will need to access the Council's case recording system to information gather and input case records. You will need a good understanding of the Occupational Therapy team and the criteria for service provision and training will be provided to support this.

This post is 5 days a week Monday to Friday.

We will provide you with the necessary training and support to enable you to successfully undertake the role and on-going training will be provided as the service continues to develop.
