Job Overview
Provide quality, holistic care to palliative and end‑of‑life patients in their own home, supporting their family and carers. The service operates 7 days a week, 365 days a year to enable patients to remain at home if desired. The role is integrated into St Giles community services provision and requires a flexible approach to meet the holistic needs of patients and carers.
Responsibilities
- Deliver personal care visits, clinical interventions, observations, and emotional support to patients and carers.
- Provide respite sits as required.
- Work under the supervision and delegation of an appropriate Registered Health Care Professional, following Hospices policies and procedures.
- Mostly work in the community, but support delivery on the in‑patient unit at the Whittington site or other hospice locations as team pressures dictate.
Qualifications
- Hold a Level 3 Health and Social Care Qualification (NVQ, Apprenticeship, or Diploma).
