Overview
Provide family history assessment to patients with potentially increased risk of cancer within the innovative Virtual Family History Clinic service hosted by the South West Thames Centre for Genomics.
Deliver screening advice based on family history and other relevant inputs for cancer risk assessment.
Contribute to improving the patient pathway through innovative digital solutions.
Responsibilities
- Undertake competent and evidence‑based family history assessment of referred patients in line with local procedures.
- Combine family history and additional information to provide screening advice according to local and national protocols.
- Maintain, update and review clinical pathways and data collection to ensure patients receive appropriate screening and management advice.
- Collaborate with clinicians and administrators to facilitate an effective and efficient patient pathway using digital innovation, including the Family History Questionnaire Service (FHQS).
- Determine when to refer patients to inherited cancer genetic services or for active management.
- Act as an educational resource to the wider health care system and trainees in mainstream breast, endoscopy and surgical units.
- Participate in audit and innovation to improve patient pathways and support service evaluation.
- Assist with the clinical aspects of the Virtual Family History Clinic service, including review and evaluation.
