We are looking for a highly motivated, well organised, enthusiastic individual to work flexibly as Discharge Facilitator within Surgery. You will be responsible for improving the patient's experience and supporting effective, timely and safe discharges by means of providing practical support to the multidisciplinary team and nurse in charge in accordance with departmental and Trust policies and procedures.

Responsibilities

  • Discharge planning within surgical wards to reduce length of stay, delayed transfers of care and increase the number of timely discharges.
  • Ensure the discharge planning process is responsive to the needs of patients and their carers by providing supportive discharge care.
  • Be visible at ward level to ensure discharge planning is proactive and timely and that patients are prepared and supported for their expected discharge day and time.
  • Daily chasing of the active request for package of care, clerking of new patient and weekly/daily discussion with multi-professional team regarding planning for safe discharge.
