The predicted salary is between 30000 - 42000 £ per year.
You will be responsible for improving the patient's experience and supporting effective, timely and safe discharges by providing practical support to the multidisciplinary team and nurse in charge in accordance with departmental and Trust policies and procedures. The post holder will undertake practical support for discharge planning within surgical wards to reduce length of stay, delayed transfers of care and increase the number of timely discharges.
The post holder will ensure the discharge planning process is responsive to the needs of patients and their carers by providing supportive discharge care. The post holder will be visible at ward level to ensure that discharge planning is proactive and timely and that patients are prepared and supported for their expected discharge day and time. Daily chasing of the active request for package of care, clerking of new patient and weekly/daily discussions with the multi-professional team regarding planning for safe discharge.
