St Elizabeth Hospice is seeking a Community Care Coordinator to join our dedicated team in Ipswich. You will work alongside clinical staff to coordinate patient services within our palliative care hub, acting as the first contact for our 24/7 OneCall service and supporting a shift pattern across 8am–8pm, seven days a week.
Full training will be provided, and we emphasise safeguarding and compassionate care across home, community, hospital, and hospice settings.
