We\’re an independent charity at the heart of our local community, offering free, flexible and person-centred palliative care and support to our patients and their families.Our Hospice at Home/ Access Team provides specialist advice, guidance and support to patients and their families in the community, wherever they call home. This role is a rational post, involving working across all three teams within the service. Part of the Hospice at Home/ Access Team Registered Nurse role involves working within our Access Team – the single point of access to support our patients and families, to triage the support they need and signpost to the help that we can provide. It also involves working within our Community Palliative Care team, supporting patients and families through our face to face, telephone advisory service. Lastly it also incorporates working within our Virtual Ward service, providing an enhanced level of hands-on care and support to patients within their homes. As well as advising on symptom control, medicines and treatments, our nurses ensure people are getting the practical help they need, from agencies such as Social Work Home Care. An exciting opportunity has arisen for a caring and compassionate registered nurse to join our Hospice at Home / Access Team Service. This is a rotational post, required to work across all three teams as required. The first year will be to work within our Virtual Ward Hospice Team for a period of one year, and laterally across the Hospice at Home team, our Access Team. You’ll be a Registered Nurse with excellent communication skills and an interest in palliative care, working within the community. The Registered Nurse will be responsible for ongoing holistic assessment and planning of care to meet the patients and carers practical, psychological, spiritual and social needs. The post at times may require you to support Inpatient Unit with sickness absence cover. A valid driving licence and use of a car is essential for this post.
