Part-time, Permanent 30 hours per week

Starting Salary £32,873 WTE, per annum, pro rata (Based on 37.5 hours per week) (rising to £34,254 pro rata after 12 months satisfactory performance review)

We are looking for an enthusiastic, approachable, organised and compassionate individual to join our AHP Team as a Rehabilitation Assistant. Working as a key member of our palliative rehabilitation service, you will support qualified therapists to implement rehabilitation plans aimed at optimising function, engagement in activity and quality of life for our patients.

You will help people at every stage of their palliative care needs from when they first become involved with hospice services and are wanting to remain physically active to supporting them through deterioration and change in function, right up until they reach the end of their lives.

You will predominantly be working with people in their own homes but some work across the In Patient and Wellbeing services will also be required. You will be delegated appropriate tasks by therapists to deliver a high standard of service to patients with specialist palliative care needs.

You will also be expected to work with a degree of autonomy and manage your own caseload. You will play an active role in service development and delivery, supporting colleagues and the AHP Team Lead to maintain excellent standards, optimisation of care and ensuring our care and services remain innovative and person centred.

The ability to work collaboratively as part of a multidisciplinary team and communicate effectively with patients, families, and healthcare professionals is essential.
