Clinical Pharmacist in Oxford

St Clements Surgery in Oxford is seeking a permanent Clinical Pharmacist to lead its medicines optimisation strategy. As a frontline clinical leader in this research-active training practice, you will work within a total triage system that ensures a structured, clinically assessed workload. Your primary focus will include conducting structured medication reviews for complex patients, managing chronic diseases like Diabetes, and overseeing weekly ward rounds for two local care homes.

The role involves high-level clinical workflow management, such as reconciling hospital discharges and participating in multidisciplinary virtual ward rounds alongside GP mentors. To qualify, you must be a GPhC-registered Independent Prescriber with at least 12 months of experience in a UK General Practice setting and proficiency in EMIS Web and Docman.

In return, the practice offers a salary of £50,000–£56,000 (40 hour FTE) for a 30-hour week, full NHS pension enrollment, and dedicated time for professional development. St Clements provides a supportive, learning-focused environment that actively encourages pharmacists to pursue Advanced Clinical Practitioner status.

As the Clinical Pharmacist at St Clements Surgery, you will serve as a frontline clinical leader responsible for the practice's medicines optimisation and safety strategies. Operating at the top of your license as an independent prescriber, you will manage complex caseloads through the delivery of structured medication reviews, specifically targeting patients with polypharmacy, frailty, and multi-morbidity.

Your role is central to the clinical workflow, involving the management of high-level prescribing transitions and the reconciliation of medication following hospital discharges. You will take a proactive lead in chronic disease management and oversee pharmaceutical care for residents in local care homes through dedicated weekly ward rounds. As a core member of a research-active training practice, you will work within a multidisciplinary environment, collaborating in virtual ward rounds and clinical meetings to drive evidence-based improvements in patient care. • Structured Medication Reviews (SMRs): Lead on SMRs for patients with polypharmacy, frailty, and multi-morbidity, focusing on ICB prescribing priorities to reduce problematic polypharmacy and improve patient safety.
• Chronic Disease & QOF Management: Take a primary role in meeting Quality and Outcomes Framework (QOF) targets, with a specific focus on Diabetes management, including insulin regimes, GLP-1 monitoring, and lipid optimisation.
• Clinical Workflow (PWS & Docman): Manage the Prescription Workflow (PWS) for safe repeat prescribing and lead the reconciliation of hospital discharge summaries via Docman.
• Care Home Leadership: Contribute to weekly Care Home Ward Rounds for two local homes, providing proactive clinical reviews and pharmacy advice to care home staff.
• Integrated Multi-Disciplinary Working: Act as a core member of the weekly Virtual Ward Round (VWR) and Diabetes MDT, collaborating with District Nurses and allied health professionals.

• GPhC Registered Pharmacist with a clear record of professional conduct.
• Minimum 12 months of experience in a UK General Practice setting.
• Expertise in EMIS Web and Docman.
• Strong understanding of clinical governance and the SEA process.
• Completion of (or enrolment in) CPPE Primary Care Pathways or ACP training.
• Proven experience in geriatrics or care home support.
• Specialist interest in Diabetes, Respiratory, or Hypertension.
• Experience in conducting clinical audits that lead to measurable safety improvements.
