Deliver safe, holistic, evidence‑based nursing care for patients with complex palliative needs, working closely with the wider MDT (medical, pharmacy, AHPs, family support, and volunteers).

Assess, plan, implement and evaluate individualized care plans focused on comfort, dignity and patient/family goals.

Administer and monitor medicines in line with hospice policies and NMC standards, escalating changes in condition appropriately.

Communicate sensitively with patients and those important to them, promoting shared decision‑making and advance care planning.

Maintain accurate, timely documentation on the electronic patient record system and contribute to quality improvement, incident reporting and learning.

Support safe discharge planning and smooth transitions of care across services.

Responsibilities

  • Deliver safe, holistic, evidence‑based nursing care for patients with complex palliative needs, working with the wider MDT.
  • Assess, plan, implement and evaluate individualized care plans focused on comfort, dignity and patient/family goals.
  • Administer and monitor medicines in line with hospice policies and NMC standards, escalating changes in condition appropriately.
  • Communicate sensitively with patients and those important to them, promoting shared decision‑making and advance care planning.
  • Maintain accurate, timely documentation on the electronic patient record system and contribute to quality improvement, incident reporting and learning.
  • Support safe discharge planning and smooth transitions of care across services.
  • Work within the hospice and wider multidisciplinary team, take lead on discharge planning, and deputise in the absence of a senior person within limits of authority.
  • Be able to communicate effectively and sensitively with patients, families and other health care professionals.
  • Promptly report adverse events, accidents, near misses and errors via the DATIX system.
  • Maintain and update own clinical skills, attend education and training, and participate in the annual personal development plan.
  • Comply with registration requirements of the NMC, possess working knowledge of hospice policies and procedures, uphold confidentiality, and promote equality and diversity.

Qualifications

  • Registered Nurse on the NMC register part 1 or 12.
  • Additional health/social care qualifications at diploma level.

Experience

  • Provision of care in an acute setting.
  • Evidence of professional development.
  • Proficient written and verbal communication in English.
  • Basic understanding of drugs commonly used in palliative care/end‑of‑life care.
  • Self‑motivation and initiative.
  • Professional, sensitive communication style and good IT skills.
  • Able to prioritise and manage workload effectively while working flexibly and as part of a team.
  • Emotional resilience to work effectively in a palliative care setting and cope with the care of patients who are dying and after death.
  • Good moving and handling skills.
  • Maintaining confidentiality and ensuring colleagues uphold the same standard.
  • Promoting equality and diversity.
  • Experience providing palliative care in other settings.
  • Good knowledge of pain and symptom management.
  • Understanding of health and safety issues in a clinical environment.
  • Good understanding of the Mental Capacity Act 2005.
