About the role
To work as part of Southern Hospice Group multi-disciplinary team providing high quality Palliative and End of Life nursing care for patients and their families in accordance with the NMC guidance and Southern Hospice Group policies and procedures.
You'll be
• Partner with District Nursing teams and Continuing Healthcare funded agencies to support the practical care of patients at end of life in the community
• Assess and monitor changes in a patient’s condition, advising on complex symptom control measures and implementing changes according to best practice and community and/or hospice policies
• Provide effective and sensitive communication between all community and hospice professionals, as well as patients, carers and relatives
• Administer medicines within assessed competency and hospice guidelines according to the needs of the patient
• Work with patients and families to create management plans addressing their individual physical, psychosocial and spiritual needs reflecting the holistic principles of palliative care and promoting dignity
