Overview

An Occupational Therapist is invited to join our community-based Specialist Palliative care team in the East of the County.
The role offers opportunities to grow the Occupational Therapy service, collaborate with the wider MDT, and support patients to identify goals and live well with their palliative diagnosis and end-of-life care.
Work is undertaken both in patients' homes and in our wellbeing hubs that provide a non-clinical backdrop to individualised care.

A flexible, adaptable approach is essential, and you will be supported with professional development.
St Barnabas Hospice has an established AHP service with regular support and contact.
Responsibilities
- Deliver individualised therapy to patients as part of the palliative care team, within the hospice setting or at patients’ homes.
- Work autonomously to support patients to identify goals and live well with their palliative diagnosis, contributing to end-of-life care planning.
- Collaborate with a multidisciplinary team to coordinate patient care and ensure holistic support.
- Engage with patients in various settings, including wellbeing hubs, to provide non-clinical, person-centred care.
Qualifications
- A qualified Occupational Therapist with HCPC registration.
- Experience in palliative or community care settings is beneficial.
- Demonstrated passion and enthusiasm for the profession.
