The predicted salary is between 36000 - 60000 £ per year.
Working within a team located across Leeds, you will support and provide interventions to people affected by alcohol, within a range of community settings. This will be an exciting opportunity to address unmet needs of people who have difficulties engaging with conventional treatment systems. You will be responsive and complete timely community-based assessments, collaborating with community providers to ensure accessible treatment options. Support will include preparation for those wishing to detox from alcohol whether this be through a residential setting or a community detox pathway. Our aim is to ensure that the service user and their family have access to appropriate care which is coordinated in line with a person‑centred approach.
Main Duties of the Job
- Work closely with the Team Manager and be responsible for your own caseload of service users.
- Organise face‑to‑face appointments and deliver therapeutic sessions including safe alcohol reduction plans, risk assessments, and harm reduction/relapse prevention work.
- Link in with GPs, Healthcare Assistants, Hospital Teams and Rehab Facilities to work holistically in supporting service users to achieve their planned treatment goals.
- Use your own initiative to identify unmet needs within our service and work collaboratively with partnership agencies.
- You will be fully supported by the Service Manager, Team Manager and Senior Detox Practitioner to deliver positive outcomes for your service users.
