We're now looking for a Band 7 Occupational Therapist to join us as a key contributor to our discharge and transition planning function. This is a brilliant opportunity to be part of a service that's evolving fast, where your clinical expertise will help shape smoother, safer and more consistent patient journeys.

Main duties of the job

  • Provide advanced OT input into complex discharge planning
  • Support the development of structured transitional programmes
  • Work closely with MDTs and external partners to unblock barriers and improve flow
  • Contribute to service improvement projects focused on progression and discharge
  • Supervise and develop the OT workforce in discharge-focused practice
  • Lead on outcome measurement and evidence-based pathway development

Who You Are

A confident, skilled OT with strong experience in mental health or secure settings, excellent clinical reasoning, and a passion for improving patient pathways and transitional care.

Job responsibilities

You will contribute to a multi-disciplinary clinical team, taking responsibility for a clinical caseload, and offering OT specific assessment and treatment to groups and individuals within your care. Strong communication skills are essential, both written and verbal, with the ability to develop good professional relationships with your clinical team and the wider professional team.

You will be able to show a good understanding of occupational therapy within inpatient services; demonstrating the hope occupational therapists bring to the lives of those in our care. You will also be able to share your experience of applying the OT process to achieve positive patient outcomes and demonstrate how you promote the profession within your clinical team.
