We are now looking for a Band 7 Occupational Therapist to join us as a key contributor to our discharge and transition planning function. Main duties of the job
- Provide advanced OT input into complex discharge planning
- Support the development of structured transitional programmes
- Work closely with MDTs and external partners to unblock barriers and improve flow
- Contribute to service improvement projects focused on progression and discharge
- Supervise and develop the OT workforce in discharge focused practice
- Lead on outcome measurement and evidence based pathway development
