We are seeking a compassionate and proactive Care Navigator to join our busy GP practice team. This is a key front-line role, supporting patients to access the right care, at the right time, from the right service. You will help patients navigate primary care services, community resources and external agencies, ensuring they receive appropriate, timely and coordinated support. Working closely with GPs, nurses, and the wider multidisciplinary team, you will act as a central point of contact for patients, helping to manage enquiries, signpost effectively, and improve overall patient experience. The role requires excellent communication skills, sound judgement, strong organisational ability, and a calm, professional approach in a fast-paced environment. Main duties of the job

  • Act as the first point of contact for patient enquiries, gathering relevant information sensitively and accurately.
  • Signpost patients to appropriate clinicians, services or community support in line with practice protocols.
  • Support care coordination for patients with long-term conditions and complex needs.
  • Process and manage appointment requests in line with triage guidance.
  • Liaise with the wider multidisciplinary team to ensure seamless patient journeys.
  • Maintain accurate clinical records and update patient information on the clinical system.
  • Contribute to improving access, patient flow and overall patient experience.

Job responsibilities

  • You will act as a first point of contact for patients and external agencies, responding to enquiries professionally and sensitively, whether in person, by telephone or electronically.
  • You will gather relevant information, care navigate in line with practice protocols, and ensure patients are directed to the most appropriate clinician or service.
  • You will also support home visit requests and liaise with community and secondary care providers where required.
  • You will manage appointment systems effectively, including booking, amending and cancelling appointments, processing recalls and supporting demand management processes.
  • This includes recording accurate information on the clinical system, supporting long-term condition monitoring and ensuring patients receive timely follow-up.
  • The role also includes general administrative duties such as processing incoming and outgoing correspondence, scanning and coding documents, maintaining accurate patient records, and providing clerical support to the wider team.
  • You will contribute to maintaining smooth day-to-day operations within the practice and undertake mandatory training as required.

Person Specification
