Experienced, motivated Practice Nurse required for Squires Lane Medical Practice. We are a proudly independent and friendly Doctor surgery. We are a dynamic team of doctors, practitioners, nurses and administrators. The Practice Nurse role is vital and varied and the successful candidate would ideally be able to undertake a full range of core clinical competencies as identified in the job description. We are looking for that exceptional Practice Nurse who is driven to ensuring excellent care to our patients.

Main duties of the job

The Practice Nurse is responsible for ensuring safe/effective nursing care to patients. Clinically, the focus is the delivery of evidence-based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients. Experience would be desirable in practice/community nursing, chronic disease management and an ability to work under pressure. Ideally you will have qualifications/experience in Asthma, Diabetes, CHD, Family Planning and cytology. Independent/Supplementary prescribing would be an advantage.

Job responsibilities

To work within Squires Lane Medical Practice participating in a comprehensive health promotion, health screening, and illness prevention programme. To deliver quality nursing care in accordance with the relevant practice protocols and in line with the NMC Code of Professional Conduct. To supervise the work of the Health Care Assistants and support their development.

PROFESSIONAL RESPONSIBILITIES

  • To maintain accurate and contemporaneous records of activities both written and computerised.
  • To promote good communications.
  • To maintain and update professional knowledge in line with the NMC Revalidation guidance.
  • To participate in educational programmes within the Practice.
  • To maintain confidentiality at all times.

CLINICAL

  • To identify, assess and discuss patients' problems or needs with the patients, relatives/carers and other Primary Health Care Team members and to devise a management plan and monitor the outcome.
  • To provide health education, screening, monitoring and nursing interventions to specified groups which will include patients newly registered to the Practice and patients with long term health problems such as diabetes, asthma, coronary heart disease, frailty.
  • To assist in achieving health promotional targets to include cervical cytology and immunisation and vaccination programmes.
  • To help develop and implement Chronic Disease Management Clinics in disease areas such as Asthma, Diabetes, COPD, Hypertension, Epilepsy, Mental Health, IGR Monitoring, Rheumatoid Arthritis etc.
  • To assist the doctors, if required.
  • To be familiar with the use of peak flow meters and nebulisers.
  • To be familiar with the procedures necessary for carrying out ECGs and monitoring blood glucose.
  • To develop advanced skills in Diabetes Management and/or Management of Respiratory conditions.
  • To be responsible for the counselling and the nursing advice to patients.
  • To help in the recognition of Mental Health problems, and to refer on to the appropriate resources.

Administration and Miscellaneous

  • To share responsibility for the management and stock control of the vaccinations and equipment.
  • Keeping an up-to-date register of all drugs and injections available to the practice, in particular controlled drugs.
  • To maintain good lines of communication with management.
  • To promptly notify the Practice Manager or her deputy of any absence.
  • To book annual leave via the Practice Manager.
  • To observe health and safety rules as laid down by the practice.
  • To record all clinical activity and health promotion data in the practice computer system.

  • NMC Registered Nurse.
  • Evidence of post registration training.
  • Continued personal development.
  • Clinical observations.

  • LTC Travel immunisations.
  • Childhood Immunisations & Vaccinations.
  • Experience/Diploma in at least 1 or 2 CDM areas e.g. Diabetes, CHD, COPD, Asthma.
  • Cervical Cytology.
  • Communication skills verbal and written.
  • Well-motivated.
  • Positive attitude to work and willingness to learn.
  • Able to work under pressure.
  • Ability to manage own workload and work independently.
  • Ability to support and supervise junior staff as required.
  • Chronic disease qualifications/Experience of community working and domiciliary visits.
  • Mentorship of students, experience of teaching.
  • Experience using EMIS systems.
  • ECG.
