Responsibilities include working within the policy framework of Springhill Hospice in its provision of specialist palliative and supportive care by supporting the team in ensuring that the clinical aims and objectives of the organisation are met. Establish and maintain good working relationships and communicate with members of the multi-disciplinary team, other healthcare professionals and users of the service to ensure the delivery of high standards of effective specialist palliative and supportive care. Actively participate in the assessment, planning and delivery of a consistently high standard of specialist palliative and supportive care. Act in accordance with the Nursing and Midwifery Council Code of Conduct.
Qualifications & Requirements: We are looking to recruit a highly motivated and compassionate individual with excellent interpersonal and communication skills to join our team. The ideal candidate has experience caring for palliative care patients in either a specialist or non-specialist setting. For the right individual, we would consider a development opportunity within the role. The successful candidate will be compassionate, resilient, and able to deliver high quality, patient-centred care. They will be confident working both independently, including lone working within the community, and collaboratively as part of a multidisciplinary team supporting patients and their families with specialist palliative and end-of-life care. A car owner with a full, clean driving licence is required.
Location: This role is based within the Community of Rochdale, Middleton and Heywood.
Springhill Hospice Services include:
- 16-bed Inpatient unit providing 24-hour medical and nursing care.
- Hospice at Home Service for people who want to end their days in their own home by providing the support and symptom management they need.
- Wellbeing Support Service operating Monday to Thursday, offering sessions both within the Hospice and in community settings, with programmes for wellbeing, symptom self-management and advance care planning.
- Community Service providing specialist palliative care and support to patients and families in their own homes, including a Night Sitting service.
- 24-hour Palliative Care Advice Line for patients, families, carers and health professionals.
- Bereavement Support for people who are experiencing difficulty in their grieving process following the loss of a loved one.
- Counselling Services for patients, carers and relatives experiencing psychological and emotional distress due to their own life-shortening illness or the illness of someone significant to them.
- Education and Training Centre providing specialist courses in palliative care and a range of other training programmes for Hospice staff and local health and social care professionals.
