We’ve been in the care business a long while, with over 50 years experience of working in Leeds, Yorkshire and the North East. Following a successful tender win with Leeds City Council, under the Community Health and Well-being Service (CHWS) contract, we are looking for dedicated, kind, compassionate care professionals who are passionate about supporting and caring for people so they can continue to live at home, and in promoting their independence, wellbeing and community inclusion.
Working as part of the dedicated CHWS care teams in West Leeds (Armley, Bramley, Wortley) you’ll be supporting individuals with their personalised care plan, with a real focus on outcomes and identifying what matters to them!
In addition to the day-to-day care and support needs of individuals, you’ll have a good understanding of the local and accessible resources available so you can signpost to services that can support with health and wellbeing needs, encourage building relationships with family, friends, and local community services.
We’d look to you to take the lead in liaising with health professionals including District Nurses, GP, and Occupational Therapists. We really value our people, and our people are best placed to know what works well and how best to support our customers. Therefore, we want to ensure we give you the best training and support to do just that and genuinely make a difference to the people we support.
