We have an exciting opportunity for a HICCS Community Care Assistant to join an organisation making a real difference in people’s lives. We are looking for a committed, reliable and experienced care support worker to join our expanding Home Independence and Crisis Support Service (HICSS). This role is within our HICCS team and is to work the following specific shift pattern:

  • Week 1 - Monday, Saturday & Sunday 7pm - 7am.
  • Week 2 - Thursday & Friday 7pm - 7am.

You will be supporting customers being discharged from hospital and returning home by ensuring the right care and support is provided during this period of transition including completing assessments, crisis interventions and emergency response, personal care, medication, and supporting with the use of daily living/rehabilitation aids.

Applicants must have a full driving license and access to their own vehicle as you will be expected to work across all areas of South Tyneside.
