This is a full-time hybrid role for a Clinical Case Manager based in Newcastle Upon Tyne. The Clinical Case Manager will be responsible for planning, coordinating and implementing case management services, ensuring the delivery of comprehensive support that meets the individual's health and social care needs.
Responsibilities include:
- Conducting assessments
- Developing care plans
- Collaborating with multidisciplinary teams
- Providing emotional and practical support
- Maintaining accurate case records to ensure high-quality outcomes for clients
Qualifications:
- Proficiency in Social Work, Social Services, and managing the social care needs of clients
- Experience and skill in Case Management and providing Case Management Services
- Understanding of and working knowledge within the Mental Health sector is highly desirable
- Capacity to work collaboratively as part of a multidisciplinary team, alongside excellent communication and interpersonal skills
- Organizational skills for maintaining accurate and compliant documentation
- Ability to manage a caseload in a person-centered, compassionate, and professional manner
- Relevant qualification in Social Work, a related field, or significant experience in case management and/or social services
- Experience in working within health and social care settings is a strong asset
- Knowledge of relevant legislation and CQC standards is a plus
